Drop Shipments

As a registered provider, you have the option to drop ship a SIBO Test kit to your patients.

To order a drop shipment, simply complete a lab requisition and fax it to our office at (425) 391-8091.  Please specify the shipping address in the “Patient Demographics” section of the lab requisition.  Receipt of the completed lab requisition will trigger the drop shipment within one business day.  Please note that all sections of the laboratory requisition must be completed to avoid delays in the outbound shipment.  

If ordering a SIBO kit containing lactulose, please fax both a completed laboratory requisition as well as a prescription for lactulose (see sample below).  Orders received without a lactulose prescription will be delayed until the prescription is received.
