ORDER A SIBO TEST FOR A PATIENT – SEATTLE, WA

As a registered provider, you may order a SIBO test kit for your patients.

To order a SIBO test and have the test kit shipped directly to your patient, simply complete a lab requisition and fax it to our office at (425) 391-8091.  Please specify the shipping address in the “Patient Demographics” section of the lab requisition.  Receipt of the completed lab requisition will trigger an outreach call to the patient to verify the patient’s shipping address within 24 hours.  Please note that all sections of the laboratory requisition must be completed to avoid delays in the outbound shipment.  

If ordering a SIBO kit containing lactulose, providers in Washington should fax both a completed laboratory requisition as well as a prescription for lactulose (see sample lactulose prescription below).  Orders from providers in Washington that are received without a lactulose prescription will be delayed until the prescription is obtained from the provider.

For providers outside of Washington who prefer the use of lactulose instead of glucose, providers must select “SIBO without lactulose” on the laboratory requisition and then send the a prescription for the patient to the patient’s preferred pharmacy.  The patient will receive a test kit without substrate and will use the lactulose from the local pharmacy in conjunction with the test kit we send.
