This section is for providers only. Providers, please complete the both sections of the form below, then click “Submit”. You will receive a confirmation email after your information has been successfully submitted. Once your registration has been submitted, we will validate your medical credentials and create your account, which is usually completed within one business day. Once your registration is complete, we will send an email to the address you provide in the form below with your account details and a summary of our services.
Important: Please note that two substrates are available in the at-home SIBO kits: lactulose and glucose. Because lactulose is regulated by the FDA as a legend drug, it is only available with a prescription from a licensed health care provider. Please check with your local state licensing board to determine if you have prescriptive rights for legend drugs before registering if you plan to prescribe SIBO test kits that contain lactulose as the substrate.
